The following members or registrants have been found to be in breach of Swim Ontario polices and procedures or the Swim Ontario Comprehensive Code of Conduct.
- Kenora Swimming Sharks - was found to be in breach of Swim Ontario Policies and Procedures by failing to register swimmers from within their club. Further the club breached policy and procedure in allowing these non-registered swimmers access and entry to competition on two separate occasions. As a result of these violations, KSS received a period of suspension of their competition sanctioning/hosting privileges for a period of 30 days (December 12, 2016 to January 11, 2017 inclusive) and was fined the amount of $1,400.00.
- Richmond Hill Aquatic Club - was found to be in breach of Swim Ontario Polices and Procedures by failing to register all swimmers from within their club in a timely manner, or at all. As a result of this violation, RHAC was fined the amount of $10,600.00
Mr. Santino Curro - was found to have contravened the Swim Ontario Comprehensive Code of Conduct and Ethics Clause 3 and Clause 13 (l) and (m). Mr. Curro was suspended from his officiating duties from February 17, 2016 through March 18, 2016 inclusive.
